When a person approves the document the workflow sets the document as approved.

I've used the out of the box approval workflow to approve documents when they are created or modified.

I hope this answers your question.

Nobody has done this or thought about it? (Would have thought this would be a fairly common scenario. )

Use the following procedure to create an .accde file. This procedure leaves the original .accdb file unmodified.

On the Save As area, under Save Database As, select Make ACCDE and then click Save As.
